7-12 Discipleship Group (optional)

Please let us know the names of any of your children, grades 7-12, who would like to participate in Discipleship Group on Monday Nights, 6-7:30pm at St. Patrick's Convent. There is no fee for discipleship.

Additional Information

K-6 Sunday - A minimum 12 families will need to register for a K-6 Sunday location in order to hold class there - if that benchmark is not met, the ones who have registered will be asked to select a different option. K-6 Sunday families, please sign.

Please list other adults who have your permission to pick up your child. Include full name, relationship and cell number. Please write NA if there is no other adult who may pick up your child.

Photo and Media Permission. From time to time we will use photos from Faith Formation and Youth Ministry on social media and in print media, such as, but not limited to, the Spirit of Hope Facebook page, the SOH Parish website, the Catholic Sun and the bulletin. This includes photos taken during activities in a classroom setting, in the convent, or at an offsite activity. In the case of Facebook, Confirmation and First Communion Masses may be livestreamed. If names are listed, your child's full name will never be disclosed, only the first name will be used.

All families pay the registration fee. Families who have children receiving their First Communion or Confirmation have the addtional Sacrament fee.
